What Documents Do I Bring?
Before visiting the DMV, please review the transaction types below to ensure you have the correct documentation.
Please Note:
- Vehicle Types: These requirements only apply to standard passenger cars and light trucks. For all other vehicle types, please view our full list of vehicles.
- Deceased Owners: If you are handling a vehicle belonging to a deceased individual, please review our guide on survivorship affidavits.
Note: While these are the standard documents required, some unique transactions may demand additional paperwork.
Vehicle - Utah
Utah Title and Registration:
Required for All Transactions:
- Valid government-issued ID (for all owners listed on the application)
- Utah Title or a Utah Duplicate Title Application
- Must include the previous owner’s signature(s) releasing interest.
- Bill of Sale
- Payment: To calculate how much your specific transaction will cost, please use our online Fee Estimator.
Required Only for Specific Situations:
- Odometer Disclosure Statement: Required if the passenger vehicle is model year 2011 or newer.
- Lien Release: Required if the current title shows an active lien holder.
- Form TC-656, Utah Title Application: Required if the new owner cannot be physically present at the time of transfer (this applies to virtual appointments as well).
- Leasing Power of Attorney: Required from the leasing company if the vehicle is currently leased.
- Emission Inspection: Required depending on your vehicle's year and county of registration.
- Safety Inspection: Required only if you are changing the vehicle brand or registering a street-legal ATV.

- Safety Inspection: Required only if you registering a street-legal ATV.
- VIN Inspection: Required only if the vehicle is coming from out of state and an emission inspection is not required.
- Leasing Power of Attorney: Required from the leasing company if the vehicle is currently leased.
- Vehicle Information: Your current registration, your renewal notice, or simply your license plate number and Vehicle
Identification Number (VIN). - Emission Inspection: If required for your vehicle's model year and county of registration.
- Payment: Cash, check, or card to cover renewal fees and taxes.

If your vehicle registration has been revoked or suspended, you must provide your vehicle information (license plate number or VIN) along with the specific documentation required for your situation below:
- If Revoked for No Insurance:
- Current proof of valid Utah liability auto insurance.
- If Suspended for Emissions Issues:
- Clearance documentation from your county health department proving the vehicle has passed inspection or the issue is resolved.
- If Tied to a Driver's License Issue:
- A clearance letter or documentation from the Utah Driver License Division (DLD) showing that your driving privilege status has been resolved.
If your vehicle’s paperwork or physical tags are lost, stolen, or damaged, you can obtain replacements by providing the specific items
below.
Replacement Title
- Required Documents:
- Utah Duplicate Title Application: Fully completed with vehicle and ownership details.
- Valid Government-Issued ID: Required if you are completing the transaction yourself. If a representative is bringing
the paperwork on your behalf, your government ID information must be filled out on Form TC-656. - Form TC-656, Utah Title Application: Required if the owner cannot be physically present (this includes virtual
appointments).
- Fee: $6.00
Replacement Decal/Sticker
- Requirements: Vehicle information (plate number or VIN).
- Fee: $5.00
Replacement License Plates
Requirements: Vehicle information (plate number or VIN).
Fee: Costs vary depending on the style you select. You can view our list of plate types for specific pricing.
Replacement Registration Card
Requirements: Vehicle information (plate number or VIN).
Fee: $4.00
Vehicle - Out of State
Out of State Title and Registration

Insufficient Evidence of Ownership
Insufficient Evidence of Ownership - Select this if you need to register a vehicle but lack any standard ownership documentation.

Disabled Placards
Placard Fees: There is no fee for standard paper disabled parking placards. If you choose to upgrade to a durable plastic permanent placard, the fee is $2.50 per placard. (Note: Durable plastic upgrades are not available for temporary placards).
New Placards
- Form TC-842, Disabled Person and Physician Disability Certification: Must be fully completed and signed by a licensed physician, physician assistant, or nurse practitioner.
- Valid Government-Issued ID: Required for the applicant. For minors, a parent or legal guardian must provide their government-issued ID.

Placard Renewals
- Valid Government-Issued ID
- Current Placard: Please bring your old placard with you to your appointment, if available.
Impound
Impound - For obtaining the necessary DMV clearance to release a vehicle from an impound lot.

Other
Select this option if you need to alter the registered gross vehicle weight (GVW) classification for a truck or commercial vehicle.
- Required Documents:
- Your current vehicle registration.
- The weight citation/ticket (if you are adjusting your weight classification as a direct result of receiving a citation).
- Payment: Fees will vary depending on the weight adjustment.
Because the "Other" category covers an incredibly wide array of unique vehicle and titling scenarios, we cannot provide a definitive checklist for every possible situation.
However, you should always bring the following baseline items to ensure we can help you:
- Valid Government-Issued ID (e.g., a driver's license or passport).
- Vehicle Information (your license plate number, VIN, or existing title paperwork).
