About This Plate
Disability Special Group
This plate is issued to individuals who qualify for disabled person’s plates for use in specially designated handicapped parking spaces. This plate may be ordered with the Life Elevated Skier, Life Elevated Arches, or plain white background, and may be personalized with up to 5 alphanumeric characters, or up to 4 for a motorcycle, by submitting TC-817, Application for Personalized and Replacement License Plates with the required personalization fee.
Requirements:
- Completed Disabled Person and Physician Certification (Form TC-842), signed by a licensed physician or nurse practitioner
- $20.50 Special Plate Fee ($16.00 Motorcycle)
- No Annual Contribution
- $7.75 Postage and Handling
